1. A 35 yr old proffesional speed-eater presents with headache, vomiting, nausea and abdominal pain. On examination he also has periorbital oedema and conjunctivits. He has recently returned from America where he came 2nd in a hot-dog speed-eating contest. He thinks some of the meat may not have been cooked properly. A gastrocnemius biopsy was taken to confirm the diagnosis.

 

 

2. A 10 yr old boy presents with fever, malaise, urticaria, myalgia, nausea, vomiting, diarrhoea and a cough. On examination he has generalized lymphadenopathy, hepatosplenomegaly and a marked eosinophilia. He has recently returned from a holiday in Egypt where he went swimming in a river. He said that he had an itchy patch on his leg before he started to feel unwell.

 

 

3. A 23 yr old medical student presents with a 1 week hx of headache, fever, malaise, rigors and sweats, diarrhoea and vomiting. On examination she is anaemic, jaundiced, tachypnoeic and has splenomegaly. She has recently returned from Africa where she spent her elective. She thinks her fever gets worse every 3 days and reports that her condition has been rapidly deteriorating.

 

 

4. A 45 yr old businesswoman presents with fever, neck stiffness, headache, sore throat and sore eyes. On examination she has lymphadenopathy and hepatosplenomegaly. In her social hx she said that she was single but that her 8 cats gave her all the love she needed.

 

 

5. A 55 yr old zoologist has been brought to you by her husband who is worried about a gradual change in her behaviour since she came back from an expedition to the Congo five months ago. She has been getting increasing forgetful and confused and sleeps lots during the day. On examination she has cervical lymphadenopathy, heptaosplenomegaly and a low-grade fever.
